Disclosure
Medicall Transport LLC may obtain consumer reports and, when applicable, investigative consumer reports to evaluate eligibility to provide contracted driving or transportation services. Reports may include identity information, criminal history, driving record, license status, employment/education verification, public records, and other information permitted by law.
Authorization
I authorize Medicall Transport LLC and its authorized screening providers, consumer reporting agencies, insurers, record-service providers, and applicable government or motor vehicle agencies to obtain and review the reports described above, including my motor vehicle record (MVR), license status, restrictions, suspensions, revocations, reportable violations, accidents, and other permitted driving information. If approved, I authorize updated background or driving reports during the contractor relationship to the extent permitted by law for safety, eligibility, insurance, compliance, or contractual purposes.
